What it is, How it works

Hair testing is recognized as an effective instrument for identifying drug and alcohol consumption. A 1.5-inch segment of roughly 200 hair strands (approximating the size of a #2 pencil) nearest the scalp will yield 100mg of hair, ideal for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advisable. We suggest weighing the specimen using a jeweler’s scale. Should scalp hair be inaccessible, an equivalent volume of body hair can be utilized. When mentioning head hair, we refer exclusively to scalp hair. In contrast, body hair pertains to all other types (facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a laboratory's system. This step involves confirming the sample is correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not covered by an electronic chain of custody system.

Screening involves a preliminary assessment for drugs of misuse. While cost-efficient for excluding drug use in most samples, a positive screen must be confirmed for judicial acceptability. Samples that yield presumptive positives during Screening require a secondary confirmation.

If presumptive positives arise in the Screening stage, additional hair is extracted from the original sample for Extraction. During this phase, drugs are recovered from hair at notably lower concentrations compared to other methods (e.g., urine or oral fluid), underscoring why hair drug screening is a challenging procedure.

Confirmation of any positive screening outcomes is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation, as needed. The total laboratory process from Accessioning to Confirmation is overseen in alignment with both the CAP (College of American Pathologists) Hair accreditation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
