A lock of 1.5 inches, consisting of about 200 hairs (approximately pencil-wide), taken from the scalp's base provides 100mg, the perfect sample for evaluation and verification. For EtG, supplementary tests, and/or panels beyond 10, it is best to use 150mg of the hair specimen. Utilizing a jeweler’s scale for weighing the sample is advisable. If scalp hair is unavailable, a comparable quantity of body hair can be utilized. The term "head hair" pertains exclusively to scalp hair. "Body hair" signifies all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory methodology for drug test processing comprises four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample into a lab’s framework. This step involves authenticating the sample's seal and shipment, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any extra data input lacking from an electronic custody chain system.

Screening consists of an initial swift assessment for drug presence. Although Screening economically excludes drug use in most cases, a positive screen needs confirmation for legal admissibility. Samples testing positive during Screening require secondary verification.

If a sample tests positive during Screening, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, substances are extracted from hair at a lower concentration than in other methods (e.g., urine or oral fluid), emphasizing that hair drug screening is the most challenging method to execute.

Confirmation of positive screening outcomes is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as needed. The entire lab sequence from Accessioning to Confirmation is scrutinized according to both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
