A 1.5-inch hair sample, consisting of about 200 strands (roughly the diameter of a #2 pencil) nearest to the scalp, provides 100mg of hair, the optimal sample size for analysis. For configurations like EtG, add-ons, or more than a 10-panel test, 150mg of hair is advisable. It’s prudent to gauge the hair weight using a jeweler’s scale. If scalp hair isn't accessible, a comparable quantity of body hair may be used. By "head hair," we mean only the scalp hair. "Body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The four key phases in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a sample into a lab’s system. This step ensures the sample was properly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and fills in any additional data entry absent from an electronic chain of custody record.

Screening is an initial rapid test for drugs of abuse. Though Screening is economical and helps eliminate drug use in most samples, a positive result must be validated for court use. Samples deemed positive in Screening must undergo further confirmation.

Upon a presumptive positive result in Screening, additional hair is extracted from the original sample for Extraction. During this phase, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluids), rendering hair drug testing the most challenging method to execute.

To confirm any positive screening result, techniques like GC/MS, GC/MS/MS, or LC/MS/MS are employed. All samples with presumptive positives are washed before confirmation as necessary. The complete lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
