For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of hair is suggested. It is advisable to use a jeweler’s scale for accuracy in weight. If scalp hair is not available, gather the same amount from body hair. Note that head hair specifies only scalp hair, whereas body hair includes all other types (facial, axillary, etc.).

Process Overview

There are four fundamental stages in the laboratory handling of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary processing of a sample within a lab system. Essential tasks include ensuring the sample’s proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry omitted by an electronic chain of custody system.

Screening provides a preliminary assessment for the presence of drugs of abuse. Any samples flagged as presumptively positive in Screening must undergo secondary confirmation.

Confirmed positives from screening are validated through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples are washed before confirmation if necessary. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
