For EtG, additional tests, and/or those beyond 10 panels, 150mg of the specimen is advisable. Using a jeweler's scale to weigh the specimen is recommended. If scalp hair is not present, an equivalent quantity of body hair may be collected. Mention of head hair refers exclusively to scalp hair, while body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

There are four primary phases in the lab processing of drug test outcomes: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample within a lab's framework. This involves confirming that the sample was correctly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry absent in an electronic chain of custody system.

Screening consists of an initial rapid inspection for substances of abuse. While it is an economical approach to exclude drug usage in most samples, a positive result requires validation to be legally defensible. Samples that test presumptively positive during Screening demand secondary confirmation.

When a sample shows a presumptive positive in the Screening phase, additional hair is taken from the original specimen and readied for Extraction. In this phase, drugs are removed from hair at a significantly reduced concentration compared to other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive Screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are purified before confirmation if necessary. The complete lab procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
