A 1.5-inch portion of approximately 200 hair strands (comparable to a #2 pencil diameter) nearest the scalp yields 100mg of hair, which is the optimal amount for both screening and confirming. For EtG, additional analyses, or tests exceeding 10 panels, a specimen of 150mg is advised. We suggest using a jeweler’s scale to measure the sample. If scalp hair cannot be obtained, an equivalent amount of body hair may be used. In this context, head hair refers exclusively to scalp hair. Other hair types, like facial or axillary, are classified as body hair.

Process Overview

The main phases in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample into a laboratory’s system. It involves verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing supplemental data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. While cost-effective for excluding drug use in the majority of samples, a positive result in Screening must be verified to stand in legal contexts. Any samples indicated as presumptively positive in Screening require further confirmation.

If a sample is deemed presumptively positive during Screening, additional hair is taken from the initial specimen and readied for Extraction. Here, drugs are extracted from hair at significantly lower concentrations than with other methodologies (e.g. urine or oral fluid), making hair drug screening the most challenging method to conduct.

Confirmation of any positive Screening outcomes is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptively positive samples are cleansed prior to confirmation when necessary. The whole laboratory sequence from Accessioning to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.
