When gathered near the scalp, hair can offer up to about a 3-month detection period for alcohol and drugs. Hair collection is straightforward, relatively challenging to tamper with, and convenient to transport.

A 1.5-inch segment of roughly 200 hair strands (resembling the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for testing and validation. For EtG, additional tests, and/or over 10-panel screens, 150mg of the specimen is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, an equivalent volume of body hair can be gathered. By head hair, we mean scalp hair solely. Body hair includes other hair forms (facial, axillary, etc.).

Procedure Outline

The primary steps in lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial sample handling into a lab's system. This involves ensuring the sample was sealed and dispatched correctly, generating a random LAN (Laboratory Accessioning Number), and fulfilling any further data input not covered by an electronic custody chain system.

Screening consists of a preliminary rapid assay for drugs of abuse. While Screening offers a budget-friendly means to dismiss drug usage for the bulk of samples, a positive indication requires validation to be admissible in legal contexts. Any samples showing presumptive positivity in Screening necessitate a subsequent confirmation.

Should a sample test presumptively positive at the Screening phase, extra hair is extracted from the original specimen and prepped for Extraction. Here, drugs are retrieved from hair at significantly reduced concentrations than in alternative methods (ex. urine or oral fluid), making hair drug screening the toughest approach to execute.

Confirmation of any affirmative screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as required. The full lab procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair classification and the accreditation of ISO / IEC 17025 standards.
