The collection process is straightforward, presenting challenges to adulteration, and is easily shippable.

Acquiring a 1.5-inch segment comprising roughly 200 strands (around the width of a #2 pencil) closest to the scalp yields 100mg, the preferred quantity for analysis. For EtG, enhancements, and/or more than 10-panel tests, 150mg is advised. We recommend that specimens be measured using a jeweler's scale. If scalp hair is not accessible, a comparable quantity of body hair may be sourced. When we refer to head hair, we're specifying scalp hair only, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary stages of drug test processing are Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample enters the lab's system. This step involves ensuring the sample’s correct sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and performing any additional entry tasks not covered by an electronic chain of custody.

Screening is a preliminary evaluation for drugs of abuse. While it offers an economical means to eliminate drug use in most samples, positive screenings must be confirmed for court admissibility. Samples testing positive during Screening require further confirmation.

If a sample tests positive in Screening, additional hair is extracted from the initial collection for Extraction. This phase extracts drugs from hair, though at lower concentrations than other methods (e.g., urine or oral fluid), making hair testing a complex undertaking.

Positive results found during Screening are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All initial positive samples are cleansed before confirmation, as appropriate. The complete lab process, from Accessioning through Confirmation, is scrutinized under both CAP (College of American Pathologists) guidelines and ISO / IEC 17025 qualifications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
