What it is, How it works

When sourced close to the scalp, hair can reveal a drug and alcohol detection window spanning up to roughly 3 months. Collecting hair is straightforward, comparatively hard to tamper with, and simple to transport.

A 1.5-inch section comprising roughly 200 hair strands (similar to the size of a #2 pencil) nearest to the scalp provides about 100mg of hair, perfect for both screening and confirmation. For EtG, additional tests, and/or assessments surpassing 10 panels, a 150mg sample is advisable. It's sensible to weigh the sample using a jeweler’s scale. If scalp hair isn't an option, an equivalent amount of body hair may be considered. When noting head hair, we specify scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The key phases in the laboratory processing of drug tests include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a specimen in a lab’s system. It incorporates verifying that the specimen was sealed and sent correctly, providing a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening entails an initial swift check for drug abuse. Although Screening is a cost-efficient method to exclude drug use in most samples, a positive indication requires confirmation to be legally valid. Samples showing presumptive positivity in Screening need a secondary confirmation.

If a sample exhibits presumptive positivity during Screening, additional hair is taken from the original sample and prepped for Extraction. Here, drugs are extracted from hair at much lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to conduct.

Confirmation of any positive screening outcome proceeds through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing before confirmation if necessary. The entire lab procedure from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair designation and also accredited to ISO / IEC 17025 standards.
