Procuring a 1.5-inch segment, approximating 200 hairs (similar to the width of a #2 pencil) from the scalp yields 100mg of hair, which is ideal for evaluation and confirmation. For EtG, supplemental assessments, or assays beyond 10 panels, 150mg of the sample is suggested. Usage of a jeweler's scale for measurement is advised. If scalp hair is not obtainable, an equivalent mass of body hair is acceptable. When discussing head hair, we specifically mean scalp hair. Conversely, body hair encompasses all other hair (facial, armpit, etc.).

Process Overview

The laboratory procedures for drug test verification consist of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase where a sample is introduced into the lab's processing system. It checks that the sample is sealed and dispatched correctly, allocates a unique LAN (Laboratory Accessioning Number), and completes any remaining data recording absent from an electronic custody documentation system.

Screening involves an initial rapid test for common drugs. Although cost-efficient for negating drug presence in many samples, positive screenings require verification for legal acceptability. Samples that preliminarily indicate positivity in Screening necessitate further confirmation.

For samples testing positive in the Screening phase, additional hair segments are taken from the initial sample for Extraction. Here, drugs are isolated from hair at lower concentrations than other methods (e.g., urine or saliva), thereby positioning hair drug testing as one of the more challenging procedures.

Confirmation of positive screening outcomes is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to confirmation, any samples with presumptive positive results may be cleansed as necessary. The complete sequence from Accessioning to Confirmation is inspected under the auspices of both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
