What it is, How it works

Hair analysis is now recognized as an effective means for identifying drug and alcohol consumption. Hair captures a comprehensive record of alcohol and various drugs by embedding biomarkers within the fibers of the expanding hair strand. When taken near the scalp, hair offers up to nearly a 3-month observation period for alcohol and other substances. Hair is straightforward to gather, relatively hard to falsify, and convenient to send.

A sample measuring 1.5 inches containing roughly 200 strands of hair (comparable to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the perfect sample size for evaluation and validation. For EtG, additional requests, and/or analyses exceeding 10 panels, 150mg of the specimen is advisable. We endorse weighing the specimen on a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair might be used. When mentioning head hair, we are referring strictly to scalp hair. Body hair pertains to all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four core stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s framework. This step includes ensuring the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data input not provided by a digital chain of custody system.

Screening entails an initial prompt inspection for substances of abuse. While Screening is an economical approach to exclude drug usage for most samples, a positive screen must be verified to be permissible in court. Any samples with a presumptive positive in Screening need a follow-up confirmation.

If a specimen shows a presumptive positive during the Screening phase, additional hair is taken from the initial specimen and prepped for Extraction. In this phase, drugs are drawn out from hair at a much lower concentration than other methodologies (e.g., urine or oral fluid), which is why hair drug testing is the most challenging method to execute.

Confirmation of any positive screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation as required. The entire lab process from Accessioning to Confirmation is supervised under the guidelines of both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
