For EtG, additions, and/or examinations beyond 10 panels, 150mg of the sample is advised. Measuring the sample on a jeweler’s scale is recommended. If scalp hair cannot be supplied, a proportional quantity of body hair is acceptable. References to head hair solely indicate scalp hair. Body hair refers to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves processing a sample into a laboratory’s system. This involves confirming that the sample was adequately sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any necessary data entry not supplied by an electronic chain of custody system.

Screening is an initial rapid test for substance abuse. While Screening is a cost-effective approach to dismiss drug use in most samples, a positive screening requires confirmation to be permissible in legal proceedings. Presumptive positive samples from Screening require additional confirmation.

For any presumptively positive sample in Screening, more hair is obtained from the initial collection and readied for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of positive screening results is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as needed. The full laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
