What it is, How it works

Hair testing has emerged as a potent instrument for the identification of drug and alcohol consumption. Hair offers an extended historical record of alcohol and other drugs by ensnaring biomarkers within the fibers of the growing hair strand. When collected near the scalp, hair can deliver up to approximately a 3-month detection period for alcohol and other drugs. Hair collection is uncomplicated, somewhat resistant to tampering, and convenient for shipping.

A sample measuring 1.5 inches comprising about 200 strands of hair (similar to the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, the optimal sample for examination and confirmation. For EtG, add-ons, and/or tests surpassing 10 panels, 150mg of the specimen is suggested. We advocate for weighing the specimen on a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair can be collected. Head hair refers exclusively to scalp hair. Body hair refers to all other types of hair (facial, axillary, etc.).

Process Overview

The four core steps involved in the laboratory handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample into a laboratory’s system. This involves verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain of custody system.

Screening encompasses an initial rapid check for drugs of misuse. Screening is a cost-effective method for ruling out drug usage on the majority of samples, yet a positive screen requires confirmation for court admissibility. Any samples presuming positive during Screening necessitate a secondary confirmation.

If a sample indicates presumptive positivity in the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. Here, drugs are extracted from hair at a much lower concentration than in alternative methodologies (e.g., urine or oral fluid), which renders hair drug screening the most challenging method to execute.

Confirmation of any positive screening result is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptively positive are washed prior to confirmation as needed. The entire laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:
