What it is, How it works

Collecting hair is straightforward, rather resistant to tampering, and convenient to send.

A 1.5-inch segment of roughly 200 hair strands (similar to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the preferred sample amount for analysis and verification. For EtG, add-ons, and/or assessments exceeding 10 panels, a 150mg sample is advisable. We suggest employing a jeweler’s scale for weight measurement. If scalp hair is inaccessible, an equivalent measure of body hair may be used. By head hair, we denote scalp hair exclusively. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the sample's initial incorporation into a laboratory system. This phase ensures the sample was appropriately sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and fulfills any supplementary data entry absent from an electronic chain of custody system.

Screening serves as an initial rapid assessment for drugs of abuse. While it is cost-efficient for excluding drug use in the majority of samples, a positive result necessitates confirmation for admissibility in legal proceedings. Any samples that screen as provisionally positive require further confirmation.

Should a sample screen positive during the initial testing, additional hair is extracted from the primary sample and readied for Extraction. In this phase, drugs are isolated from hair at substantially lower concentrations than in alternate techniques (e.g., urine or oral fluids), underscoring why hair drug screening is particularly challenging.

Verification of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All provisionally positive samples undergo washing prior to confirmation if required. The complete laboratory procedure from Accessioning through to Confirmation is conducted under the CAP (College of American Pathologists) Hair designation and the accreditation criteria of ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
