A 1.5-inch sample consisting of roughly 200 strands of hair (comparable to the thickness of a #2 pencil) from the scalp area will yield 100mg, which is ideal for both screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, 150mg is recommended. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be utilized. When we mention head hair, we specifically mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is introduced into the lab's system. This process encompasses ensuring the sample was properly sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and entering any supplementary data that an electronic chain of custody system does not provide.

Screening is a preliminary assessment for identifying drugs of abuse. Though Screening effectively rules out drug usage for most samples due to its cost-efficiency, a positive screen result needs confirmation to be legally acceptable. Any samples that are initially positive in Screening require a confirmatory follow-up.

If Screening indicates a presumptive positive, additional hair is taken from the initial sample for Extraction. This stage involves extracting drugs at a significantly lower concentration than other methods (such as urine or oral fluid), which underscores why hair drug screening is the most challenging technique to execute.

Confirmation of any positive result from Screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumed positive are cleaned before confirmation as required. The complete lab process, from Accessioning to Confirmation, is assessed under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
