What it is, How it works

Hair analysis is increasingly recognized as an effective method for identifying drug and alcohol consumption. Hair captures a long-term record of alcohol and drug use by embedding biomarkers within the fibers of the growing hair. When taken near the scalp, it can offer a detection period of up to 3 months for alcohol and drugs. Hair collection is straightforward, not easily tampered with, and simple to ship.

A 1.5-inch section of around 200 hair strands (about the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, the optimal amount for testing. For EtG, additional tests, and tests above 10 panels, 150mg is advised. Utilizing a jeweler's scale to weigh the specimen is recommended. If scalp hair isn't accessible, a similar quantity of body hair can be used. When we mention head hair, we specifically mean scalp hair. Body hair includes other types like facial and axillary hair.

Process Overview

There are four primary stages in the laboratory process for a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the sample's initial entry into a lab's system. It involves ensuring the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data not provided by an electronic chain of custody.

Screening involves a preliminary check for substance abuse. Though cost-effective and capable of excluding drug use for most specimens, a positive screen necessitates verification to be court-admissible. All samples with presumptive positive results in Screening need confirmatory testing.

If Screening yields a presumptive positive, more hair is extracted from the original sample for Extraction. During this phase, drugs are isolated from hair at much lower concentrations than in other methods (urine or oral fluid), thus making hair drug testing among the most challenging methods to execute.

Any positive screening results are validated through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a washing process before confirmation as necessary. The full laboratory procedure, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Advantages of hair drug testing:

  • Difficulty to cheat: It is very difficult to cheat a hair drug test, making the results more accurate.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
