Gathering a 1.5-inch segment, consisting of around 200 hair strands (approximately the same thickness as a #2 pencil) nearest the scalp yields 100mg of hair—an optimal specimen size for screening and verification. For tests involving EtG, additional compounds, and/or those exceeding 10 panels, a sample of 150mg is advisable. We propose using a jeweler’s scale to measure the sample. If scalp hair isn’t accessible, a comparable amount of body hair can be collected. In the context of head hair, this refers to scalp hair exclusively. Body hair encompasses all other hair types, including facial and axillary hair.

Process Overview

The laboratory procedures required to assess a drug test sample traverse four principal stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample’s initial integration into the laboratory’s workflow occurs. This phase includes validating the proper sealing and shipment of the specimen, assigning a random Laboratory Accessioning Number (LAN), and finalizing any data entry not covered by an electronic chain of custody system.

Screening executes an initial, rapid evaluation for substance abuse. Screening serves as a cost-efficient method to dismiss the presence of drugs in most samples; however, positive findings demand confirmation for them to hold legal standing. Samples deemed presumptively positive in Screening require further affirmation.

Should a sample indicate a presumptive positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. Here, drugs are isolated from hair in much smaller concentrations compared to other methods (e.g., urine or oral fluid), contributing to the unique challenge hair drug screening presents.

GC/MS, GC/MS/MS or LC/MS/MS techniques are employed for the Confirmation of any positive findings from the Screening phase. All potentially positive samples are washed before Confirmation, as necessary. The complete laboratory process, from Accessioning through to Confirmation, undergoes scrutiny under the CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 criteria.
