Its main advantage is a longer window of drug detection, up to 90 days, with an observed collection process making it hard to falsify. However, for recent drug use under 5 days, urine tests remain more precise.

What it is, How it works

Utilizing hair testing has emerged as an effective method for identifying drug and alcohol consumption. Hair strands chronicle a sustained record of substances by capturing biomarkers within the fibrous structure of the hair as it grows. When obtained close to the scalp, hair offers a detection span of up to about three months for alcohol and other substances. Hair collection is straightforward, relatively resistant to tampering, and convenient for transport.

A lock of 1.5 inches of roughly 200 hair strands (approximating the circumference of a #2 pencil) nearest to the scalp yields 100mg of hair, considered the optimal amount for testing and confirmation. For EtG, supplementary tests, or tests exceeding 10 panels, a specimen weight of 150mg is advised. We suggest measuring the specimen with a jeweler’s scale. Should scalp hair be unavailable, a similar quantity of body hair can be obtained. For head hair, we exclusively mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of drug test results encompasses four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial sample processing within a lab’s system. This entails ensuring proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not provided by an electronic chain of custody system.

Screening involves a preliminary and rapid assessment for potential drugs of misuse. Screening is a cost-effective method to exclude drug usage in most samples; however, a positive result requires confirmation to hold in a legal setting. Any samples with a presumptive positive result in Screening necessitate a secondary confirmation.

Upon obtaining a presumptive positive result during Screening, additional hair is extracted from the initial sample for Extraction. During Extraction, drugs are isolated from the hair at a much reduced concentration than other methods (e.g., urine or oral fluid), making hair drug screening the most intricate method to execute.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples testing presumptively positive undergo washing prior to confirmation, if necessary. The comprehensive lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair criteria and adherence to ISO / IEC 17025 accreditation standards.

Advantages of hair drug testing:

  • Extended detection span: Hair drug testing can reveal drug use for as long as 90 days, unlike urine tests which have a shorter detection span.
  • Challenging to falsify: Cheating a hair drug test is considerably difficult, thus enhancing result accuracy.
  • Chronological usage data: It can indicate drug use patterns over time rather than just recent usage.

Limitations:

  • Inability to identify recent usage: Drugs become detectible in hair approximately 5-7 days after use.
  • Expense: Hair drug tests tend to be costlier than other drug testing methods.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
