Collected near the scalp, hair allows for a detection timeframe of up to about three months for alcohol and drugs. The collection of hair is easy, altering it is not straightforward, and it is straightforward to transport.

A 1.5-inch section comprising roughly 200 hair strands (about the diameter of a #2 pencil) closest to the scalp yields 100mg of hair, the preferred quantity for screening and validation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is suggested. We advise using a jeweler’s scale to measure the sample's weight. If scalp hair is not available, an equivalent quantity of body hair can be gathered. When mentioning head hair, we mean scalp hair exclusively. Process Summary

The four primary stages in the laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample into a laboratory’s system. It involves verifying the correct sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not included by an electronic chain of custody system.

Screening entails an initial prompt assessment for drugs of misuse. Although Screening offers an economical way to disprove drug consumption on most samples, a positive screen requires confirmation to be admissible legally. Any samples that presumptively yield positive results in Screening need supplementary confirmation.

If a specimen presumptively tests positive at the Screening phase, more hair is taken from the original sample and arranged for Extraction. In this step, drugs are extracted from hair at notably lower concentrations compared to other procedures (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to execute.

Confirmation of any positive Screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation when necessary. The entire lab sequence from Accessioning to Confirmation undergoes examination under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
