What it is, How it works

Hair testing is now perceived as an effective instrument for identifying drug and alcohol consumption. Hair offers a comprehensive record of alcohol and drug use by encapsulating biomarkers within the fibers of the expanding hair strand. When obtained near the scalp, it can yield up to a three-month detection duration for alcohol and drugs. Collecting hair is uncomplicated, relatively resistant to tampering, and straightforward to transport.

A 1.5-inch segment of roughly 200 hair strands (approximately the size of a #2 pencil) closest to the scalp will provide 100mg of hair, the optimal sample for analysis and verification. For EtG, additional tests, and/or screenings beyond 10 panels, 150mg of the sample is preferable. We suggest using a jeweler’s scale for measuring the specimen. If scalp hair is not available, a comparable quantity of body hair may be gathered. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory’s system, including verifying that the sample was adequately sealed and transported, assigning a random Laboratory Accessioning Number (LAN), and completing any remaining data entry not covered by an electronic chain of custody system.

Screening entails a preliminary fast check for drug use. Although Screening is an economical method to exclude drug use in most samples, a positive screening needs further confirmation to be admissible in legal settings. Samples that are initially positive in Screening require subsequent confirmation.

For samples that are initially positive in the Screening phase, additional hair is extracted from the original specimen to prepare for Extraction. At this stage, drugs are removed from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to conduct.

Positive screening results are verified via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumed positive results undergo washing before confirmation as needed. The complete laboratory process, from Accessioning to Confirmation, is evaluated under both the College of American Pathologists (CAP) Hair designation and ISO / IEC 17025 standards accreditation.

Advantages of hair drug testing:

  • Prolonged detection period: Hair drug tests can identify drug use for up to 90 days, whereas urine tests have a shorter detection period.
  • Challenging to deceive: It is challenging to cheat a hair drug test, ensuring more reliable results.
  • Provides historical patterns: It offers insights into drug use over time, not just recent instances.

Limitations:

  • Incapable of detecting recent use: Drugs become detectable in hair after 5-7 days.
  • Expense: Hair drug tests are typically pricier than other testing techniques.
  • Result disparities: Variables such as hair color and personal differences in hair growth can influence drug metabolite concentration in hair.

Note: Though often termed "hair follicle tests", the actual analysis involves the hair strand, not the follicle beneath the scalp.

Why Hair Follicle Testing Beats Urine Testing

When comparing hair follicle testing to traditional urine tests in Kilauea, HI, the benefits are clear. Hair testing offers superior detection accuracy and reliability, capturing a comprehensive drug use history. Unlike urine tests, which are limited to a shorter detection window, hair tests provide a longer-term view.
