What it is, How it works

When taken close to the scalp, hair may offer a detection period of up to around 3 months for alcohol and other drugs. Hair is straightforward to collect, somewhat difficult to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 strands of hair (comparable to the diameter of a #2 pencil) nearest to the scalp provides 100mg of hair, an optimal amount for screening and confirmation. For EtG, add-ons, and/or assessments beyond 10 panels, 150mg of the sample is suggested. It's advisable to weigh the sample using a jeweler’s scale. If scalp hair is unavailable, a comparable quantity of body hair can be collected. When mentioning head hair, it refers specifically to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning features the initial reception of a sample into a laboratory’s system. This step comprises checking that the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and executing any supplementary data entry not furnished by an electronic chain of custody system.

Screening entails an initial rapid verification for drugs of abuse. Although Screening cost-effectively excludes drug usage in most samples, a positive screen requires confirmation for court admissibility. Samples with a presumptive positive result in Screening require a secondary confirmation.

If a presumptive positive is found during Screening, additional hair is extracted from the original sample and readied for Extraction. Here, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug screening is a challenging methodology.

Confirmation of positive screening outcomes is performed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as needed. The complete laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
