What it is, How it works

Hair testing is regarded as an effective means for detecting drug and alcohol use. Hair captures a long-term record of alcohol and other drugs through biomarkers embedded in the fibers of the growing hair strand. When hair samples are collected close to the scalp, they can offer a detection window of up to roughly 3 months for alcohol and other drugs. Collecting hair is straightforward, relatively challenging to tamper with, and simple to transport.

An ideal test can be obtained from a 1.5-inch sample consisting of about 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp, which provides 100mg of hair. For screenings such as EtG, additional testing, or those exceeding 10 panels, it is advised to collect 150mg of the specimen. Using a jeweler’s scale to measure the specimen's weight is recommended. In cases where scalp hair is not available, an equivalent amount of body hair can be collected instead. By head hair, we specifically mean scalp hair. Body hair includes any other type (facial, axillary, etc.).

Process Overview

The laboratory methodology for a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the first stage of processing a sample into a laboratory system. The tasks include confirming the sample's seal and shipment, assigning a random LAN (Laboratory Accessioning Number), and carrying out any extra data entry not provided by an electronic chain of custody system.

Screening involves conducting an initial rapid assessment for drugs of abuse. Although Screening offers a cost-effective method to exclude drug usage for most samples, a positive result must be validated to be considered court-admissible. Samples exhibiting a presumptively positive result during Screening require secondary confirmation.

If a sample indicates a presumptively positive result during Screening, additional hair is extracted from the primary specimen for preparation in the Extraction stage. This process removes drugs from hair at a significantly lower concentration than other methods (for example, urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of a positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples undergo washing as necessary. The whole laboratory procedure, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair approval and maintains accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
