A section of 1.5 inches consisting of roughly 200 hair strands (similar in thickness to a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal quantity for both initial screening and subsequent confirmation. For EtG, additional tests, or for tests exceeding 10 panels, a specimen weighing 150mg is suggested. It is advisable to use a jeweler’s scale to measure the sample's weight. In cases where scalp hair is inaccessible, an equivalent quantity of body hair may be used. References to head hair specifically pertain to scalp hair. Conversely, body hair includes all other hair types (e.g., facial, axillary).

Process Overview

The procedure for analyzing a drug test sample in the lab involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the preliminary processing of a sample in the laboratory environment. This step involves confirming that the sample was properly sealed and transported, assigning a LAN (Laboratory Accessioning Number) randomly, and filling in any additional data not provided by an electronic chain of custody.

Screening entails an initial rapid assessment for substances of abuse. While Screening offers an economical way to eliminate drug use in most cases, positives in screening must be verified for court use. Therefore, samples that test presumptively positive require further confirmation.

Should a sample test presumptively positive in the Screening phase, additional hair is extracted from the original sample for further preparation in the Extraction stage. Here, substances are extracted from the hair at concentrations considerably lower than those found in other methods (such as urine or oral fluid), which is why hair drug analysis is among the most challenging techniques.

Confirmation for any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that show presumptive positivity are washed before undergoing confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
