Hair testing is widely recognized as an effective method for identifying drug and alcohol consumption. Hair offers a long-term record of alcohol and drug use by embedding biomarkers within the strands of growing hair. When collected near the scalp, hair can reveal usage for up to a 3-month period. It is easy to collect, challenging to alter, and convenient to transport.
A 1.5-inch section of around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, an ideal quantity for analysis and verification. For EtG, supplementary tests, and/or panels exceeding 10, a recommendation of 150mg of hair is suggested. It is advisable to measure the sample on a jeweler’s scale. In the absence of scalp hair, an equivalent volume of body hair can be gathered. By "head hair," we mean scalp hair exclusively. "Body hair" pertains to all other types of hair (facial, axillary, and so forth).
Process Overview
There are four primary phases in laboratory processing of drug test outcomes: Accessioning, Screening, Extraction, and Confirmation.
Accessioning concerns the initial integration of a sample into a lab's system. This step verifies sample sealing and shipment, assigns a unique LAN (Laboratory Accessioning Number), and fulfills any additional data input not covered by an electronic chain of custody.
Screening is an initial assessment for drug presence. While it is cost-effective for excluding drug use in most samples, a screening result must undergo confirmation for court acceptance. Any sample with a positive screening result needs further verification.
If Screening suggests positive results, additional hair is extracted from the original sample for Extraction. During this phase, drugs are obtained from hair in much lower concentrations than other methods (like urine or oral fluid), making hair drug testing particularly complex.
Positive screening results must be verified by GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation. The entirety of the laboratory process, from Accessioning to Confirmation, is reviewed following the CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 standards.
