What it is, How it works

When collected near the scalp, hair offers a detection window of up to nearly 3 months for alcohol and various drugs. Hair sample collection is straightforward, challenging to tamper with, and easy to transport.

A 1.5-inch hair sample of around 200 strands (comparable in size to a #2 pencil) close to the scalp will yield 100mg, which is the optimal sample size for screening and verification. For EtG, additional tests, and/or examinations beyond 10 panels, 150mg of the sample is advisable. It's suggested to weigh the hair on a jeweler's scale. Should scalp hair be unavailable, an equivalent amount of body hair is acceptable. By head hair, we specifically mean scalp hair, while body hair refers to any other type (facial, axillary, etc.).

Process Overview

The laboratory procedure for a drug test result consists of four crucial stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step where a sample is integrated into a laboratory's system. This process involves confirming that the sample was appropriately sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic chain of custody system.

Screening provides an initial rapid assessment for drug abuse. Although Screening is a cost-efficient means to rule out drug use in most samples, a positive screening must be validated to be legally admissible. Any samples deemed presumptively positive during Screening require a follow-up confirmation.

Should a sample test presumptively positive during Screening, additional hair is taken from the original sample for Extraction. Drugs are extracted from hair at a lower concentration than in other methods (e.g., urine or oral fluid), which makes hair drug screening one of the most complex methodologies to execute.

The Confirmation of any presumptive positive screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are washed as required. The laboratory procedures from Accessioning to Confirmation are thoroughly inspected under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
