A 1.5-inch piece consisting of roughly 200 hair strands (similar to the size of a #2 pencil) nearest the scalp will produce 100mg of hair, which is the perfect amount for screening and confirmation. For EtG, additional testing, and/or panels exceeding ten, a 150mg specimen is advised. It is recommended to weigh the specimen using a jeweler’s scale. If scalp hair isn't accessible, a similar quantity of body hair can be used. When mentioning head hair, it refers exclusively to scalp hair. Body hair signifies all other hair types (i.e., facial, axillary, etc.).

Process Overview

The four key stages in lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample within a laboratory's apparatus. This stage ensures the sample was adequately sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and finalizes any additional data input not furnished by an electronic chain of custody system.

Screening entails a preliminary swift assessment for substance misuse. While Screening costs less to exclude drug usage in most cases, a positive result must be verified to hold up in court. Any samples that test presumptively positive during Screening demand further confirmation.

Should a sample test presumptively positive during Screening, additional hair is removed from the original sample for Extraction. At this stage, drugs are extracted from hair at a much lower concentration compared to other methods (like urine or oral fluid), hence hair drug screening is considered the most complex methodology.

Confirmation of a positive screen is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples that test presumptively positive are cleansed before confirmation as required. The full laboratory process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
