A 1.5-inch portion of about 200 hair strands (roughly the size of a #2 pencil) nearest the scalp provides 100mg of hair, the optimal sample for testing and confirmation. For EtG, add-ons, or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest weighing the specimen using a jeweler’s scale. If scalp hair is absent, an equivalent amount of body hair can be collected. When mentioning head hair, we mean scalp hair exclusively. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The four essential steps in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial handling of a sample within a laboratory's system. This involves verifying the sample was correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not supplied by an electronic chain of custody system.

Screening checks quickly for drugs of abuse. While Screening is a cost-effective method to eliminate drug usage in most samples, a positive result must be confirmed to be valid in legal settings. Samples that are presumptively positive in Screening require a secondary confirmation.

If a sample is presumptively positive during the Screening phase, more hair is extracted from the initial specimen and prepped for Extraction. In this phase, drugs are drawn from hair at a lower concentration compared to other methodologies (e.g., urine or oral fluid), which makes hair drug screening the most demanding methodology to execute.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
