To conduct an optimal hair analysis, a sample of 1.5 inches from about 200 strands close to the scalp is required, providing 100mg for screening and confirmation. For tests like EtG, or those exceeding 10 panels, 150mg of hair is advised. It's best to measure the sample using a jeweler’s scale. If scalp hair is unavailable, an equivalent sample of body hair can be used. "Head hair" strictly refers to scalp hair, whereas "body hair" encompasses all other hair types (such as facial and axillary).

Process Overview

The lab processing of a drug test includes the steps of Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the start, where a sample is integrated into the lab's system. This involves confirming that the sample was securely sealed and shipped, assigning a LAN (Laboratory Accessioning Number), and completing any additional data needed beyond what an electronic chain of custody system provides.

Screening is a preliminary assessment for drug use. It's a cost-saving method for eliminating the bulk of samples from further testing, but any positive results require confirmation to be accepted in court. A sample that screens positive must undergo confirmation testing.

Should a sample show presumptive positivity during screening, further hair is taken from the initial collection for Extraction. Here, drugs are isolated from hair at much lower levels than in other methods (like urine or oral fluid), making hair drug testing notably challenging.

Positive results from Screening are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS technologies. All presumptively positive samples are cleansed before confirmation as necessary. From Accessioning to Confirmation, the entire process is checked according to CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

    The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
