However, for detecting recent drug use, particularly within the past 5 days, the urine test is still considered the best option.

What it is, How it works

Utilizing hair testing has emerged as an effective method for identifying drug and alcohol consumption. Hair captures a long-term record of alcohol and drug involvement by embedding biomarkers in the hair strand as it grows. When collected adjacent to the scalp, hair can provide up to a 3-month detection span for alcohol and drugs. Hair collection is straightforward, relatively hard to tamper with, and convenient to transport.

A 1.5-inch sample comprising roughly 200 hair strands (around the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample size for both screening and confirmation purposes. For EtG, additional tests, or tests exceeding 10 panels, a 150mg specimen is preferable. It's advisable to weigh the specimen using a jeweler’s scale. If scalp hair is unattainable, an equivalent quantity of body hair can be collected. Head hair specifically refers to scalp hair, while body hair pertains to all other types (facial, axillary, etc.).

Process Overview

There are four key steps in the lab process of deriving a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step that involves entering a sample into a lab system. This process includes verifying the sample's sealed and proper shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any manual data entry that an electronic custody chain system doesn't cover.

Screening provides an initial rapid assessment for detecting drugs of misuse. Although Screening serves as a cost-efficient means to exclude drug usage in most samples, any positive findings must undergo confirmation to be acceptable in legal scenarios. Samples deemed presumptively positive during Screening must undergo a secondary confirmation.

If a sample is flagged as presumptively positive in the Screening phase, additional hair is extracted from the initial sample and directed for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than seen in other methodologies (e.g., urine or oral fluids), highlighting why hair screening is the most complex methodology to perform.

Any positive screening results are subjected to Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are washed as necessary before Confirmation. The whole lab workflow, from Accessioning to Confirmation, is scrutinized according to CAP (College of American Pathologists) Hair designation and, additionally, based on ISO / IEC 17025 accreditation standards.
