Hair testing is recognized as a potent means for identifying drug and alcohol consumption. Hair retains a prolonged record of substance usage by embedding biomarkers within the strands’ fibers during growth. When gathered near the scalp, hair offers a detection window of roughly three months for substances. Collecting hair is straightforward, challenging to tamper with, and convenient for shipping.
A sample measuring 1.5 inches, comprising approximately 200 hair strands (comparable to a #2 pencil’s thickness) near the scalp, yields 100mg of hair, ideal for screening and confirmation processes. For EtG, enhancements, or tests exceeding 10 panels, a 150mg specimen is advised. We suggest measuring the specimen using a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair can be used. When discussing head hair, we mean scalp hair only. Body hair includes all other types (facial, axillary, etc.).
Process Overview
The laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.
During Accessioning, a sample is initially integrated into a laboratory's system. This step verifies the sample's proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not captured by an electronic chain of custody system.
Screening involves a preliminary rapid drug abuse check. While Screening efficiently rules out drug use in most samples, a court-admissible positive screen must undergo confirmation. Samples that are presumptively positive in Screening demand a secondary confirmation.
Should the Screening stage yield a presumptively positive sample, additional hair is taken from the initial specimen for Extraction. Here, drug extraction from hair occurs at a reduced concentration compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging methodology to execute.
Confirmation of any positive screening outcome occurs through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if necessary. The complete laboratory process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
