Should scalp hair be unavailable, an equivalent portion of body hair can be gathered. "Head hair" refers solely to scalp hair, whereas "body hair" encompasses all other hair types (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test result in a laboratory includes four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary incorporation of a sample into a lab's system. It involves confirming the proper sealing and shipping of the sample, assigning it a random LAN (Laboratory Accessioning Number), and executing any supplementary data entry that an electronic chain of custody system hasn't covered.

Screening consists of a preliminary rapid examination for drug abuse. Although Screening is a cost-efficient method to eliminate drug use in most samples, any positive results need confirmation to be legally valid. Samples that test presumptively positive during Screening require further confirmation.

If Screening results are presumptively positive, additional hair from the original sample is used for Extraction. During Extraction, drugs are separated from hair at much lower concentrations compared to other methods (such as urine or oral fluid), which renders hair drug testing the most complex methodology.

Confirmation of a positive screening outcome is undertaken using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, presumptive positive samples undergo washing as necessary. The entire procedure—from Accessioning to Confirmation—is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
