What it is, How it works

It's straightforward to collect, challenging to tamper with, and convenient to transport.

A sample measuring 1.5 inches, comprising about 200 hair strands or nearly the size of a #2 pencil closest to the scalp, will provide 100mg of hair, which is optimal for both screening and confirmation. For EtG, additional measures, and tests exceeding 10 panels, a sample amounting to 150mg is advisable. We propose using a jeweler’s scale for precise measurement. Should scalp hair be scarce, an equivalent weight of body hair can be collected. The term head hair specifically indicates scalp hair, while body hair denotes all other hair varieties (facial, axillary, etc.).

Process Overview

There are four core phases in the lab handling of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial process of incorporating a sample into a lab’s records. This phase entails confirming that the sample was securely sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and executing any extra data input absent from an electronic custody system.

Screening consists of a rapid preliminary evaluation for abused drugs. Despite Screening being a budget-friendly way to disregard drug utilization in most samples, a positive result requires confirmation to be valid legally. Samples that test positive in Screening necessitate additional confirmation.

Should a sample appear positive during the Screening process, additional hair is taken from the original sample and readied for Extraction. In this phase, drugs are extracted from hair in much lower concentrations compared to other techniques (such as urine or oral fluid), making hair drug screening the most challenging technique to execute.

Any positive results from screening undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation follows the guidelines of the CAP (College of American Pathologists) Hair designation and holds accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
