A 1.5-inch portion containing around 200 strands (roughly the thickness of a #2 pencil) closest to the scalp produces 100mg of hair, deemed the optimal sample size for screening and verification. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample with a jeweler’s scale. If scalp hair isn’t accessible, an equivalent amount of body hair can be obtained. When mentioning head hair, we solely refer to scalp hair. Body hair denotes all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory workflow for processing a drug test result encompasses Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial input of a sample into a lab’s system. It includes checking that the sample was sealed and sent appropriately, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not handled by an electronic chain of custody system.

Screening provides an initial assessment for drugs of abuse. Although Screening is an economical approach to exclude drug use in most samples, a positive screen must be confirmed to hold up in court. Any samples that are presumably positive in Screening necessitate additional confirmation.

If a sample appears presumptively positive during the Screening stage, more hair is taken from the initial sample and prepped for Extraction. Here, drugs are extracted from hair at significantly lower concentrations than in other testing methods (like urine or oral fluids), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All potentially positive samples undergo washing before confirmation if needed. The complete laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
