What it is, How it works

When collected near the scalp, hair can present up to roughly a 3-month detection span for alcohol and drugs. Hair is straightforward to obtain, relatively hard to tamper with, and easy to transport.

A 1.5-inch collection of around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal sample for analysis and verification. For EtG, additional screens, and/or tests exceeding 10 panels, 150mg of the sample is advisable. We suggest using a jeweler's scale to weigh the sample. If scalp hair isn't available, an equivalent amount of body hair may be used. When mentioning head hair, it pertains exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four key phases in processing a drug test result in a laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial handling of a sample into a lab's system. This entails verifying the sample was sealed and mailed correctly, generating a random LAN (Laboratory Accessioning Number), and completing any extra data entry not supplied by an electronic chain of custody system.

Screening comprises an initial rapid assessment for drugs. While Screening is a cost-efficient method to rule out drug use for most samples, a positive screen must be confirmed for court admissibility. Any samples presumptively positive in Screening do require a secondary confirmation.

If a sample appears positive in the Screening step, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are sourced from hair at a significantly lower concentration than in other methods (for example, urine or oral fluid), making hair drug testing the most challenging methodology to execute.

Confirmation of any positive Screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicated as positive are cleansed before confirmation, if necessary. The full laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
