For EtG, supplementary elements, and/or examinations beyond 10 panels, 150mg of the sample is advised. Utilizing a jeweler’s scale to measure the sample is recommended. If scalp hair cannot be collected, a similar amount of body hair might be used. When mentioning head hair, it is specific to scalp hair alone. Body hair encompasses all other hair variants (facial, axillary, etc.).

Process Overview

The principal procedures in the lab processing of a drug testing outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a specimen into a lab’s procedures. It involves ensuring the sample was securely sealed and dispatched correctly, allocating a distinct LAN (Laboratory Accessioning Number), and completing any further data recording not supplied by an electronic chain of custody system.

Samples that are presumptively positive in Screening necessitate an additional confirming test.

If a specimen is tentatively positive at the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations than in alternative methodologies (e.g., urine or oral fluid), which is why hair drug analysis is considered the most challenging methodology to conduct.

Verification of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with preliminary positive results undergo washing before confirmation if necessary. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair specification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
