A 1.5-inch section consisting of roughly 200 hair strands (around the size of a #2 pencil) nearest to the scalp supplies 100mg of hair, which is the optimal amount for testing and result confirmation. For EtG, extra screenings, and/or tests exceeding 10 panels, a 150mg sample is suggested. It's advisable to determine the sample weight using a jeweler’s scale. Should scalp hair be unavailable, an equal measure of body hair can be gathered. When discussing head hair, we refer expressly to scalp hair. Body hair encompasses all other varieties (facial, axillary, etc.).

Process Summary

The four fundamental stages in the lab processing of drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial processing of a sample into the lab's system. It includes verifying the sample's sealing and shipment integrity, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data entry absent from an electronic chain of custody system.

Screening comprises an initial rapid test for drug misuse. Although Screening serves as a cost-effective method to exclude drug usage in most specimens, a positive screen necessitates confirmation to stand in court. Every sample deemed presumptively positive in Screening demands an additional confirmation.

If a sample yields a presumptive positive result in Screening, more hair is extracted from the original specimen and prepared for Extraction. This stage involves extracting drugs from hair at a much lower concentration compared to other methods (ex. urine or oral fluid), rendering hair drug screening the most challenging method to execute.

Confirmation of any positive screening result utilizes GC/MS, GC/MS/MS, or LC/MS/MS. All samples with a presumptive positive status undergo washing before confirmation if required. The entire lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
