What it is, How it works

If collected near the scalp, hair offers up to a 3-month period for alcohol and drug detection. It's easy to gather, difficult to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (comparable to the thickness of a #2 pencil) close to the scalp provides 100mg of hair, the perfect amount for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg is advised. We advise weighing the sample on a jeweler’s scale. If scalp hair isn't available, an equal portion of body hair can be used. When we mention head hair, we refer specifically to scalp hair. Body hair encompasses other types such as facial or axillary hair.

Process Overview

The four primary steps in processing a drug test result in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with entering a sample into the lab's system. This process confirms the sample was sealed and delivered correctly, assigns a random LAN (Laboratory Accessioning Number), and finishes any additional data entry omitted by an electronic chain of custody.

Screening is a rapid preliminary assessment for drugs of abuse. Although Screening is an economical method for eliminating drug use in most samples, a positive screening must be confirmed to be valid in court. Samples that are tentatively positive during Screening require a secondary confirmation.

If Screening suggests a positive result, additional hair is extracted from the initial sample and readied for Extraction. During Extraction, drugs are retrieved from hair at much reduced concentrations compared to other methods (e.g., urine or saliva), which is why hair drug screening is among the toughest methodologies to conduct.

Positive screening confirmations occur through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples undergo a cleaning as necessary. The comprehensive lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair criteria and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
