A 1.5-inch piece consisting of around 200 hair strands (approximately the width of a #2 pencil) closest to the scalp will supply 100mg of hair, which is the perfect amount for both screening and confirmation. For tests such as EtG, add-ons, and/or those exceeding 10 panels, a sample weighing 150mg is advised. We suggest using a jeweler’s scale for measuring the sample. In cases where scalp hair is not obtainable, an equivalent portion of body hair may be gathered. References to head hair imply scalp hair specifically. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The processing of a drug test result in a lab involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning requires the initial integration of a sample into a lab’s system. This phase involves verifying secure sealing and proper shipment of the sample, generating a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. While it is cost-effective for excluding drug use in most samples, a positive screen must be verified for court admissibility. Presumptive positives in Screening necessitate further verification.

If Screening results are presumptively positive, more hair is extracted from the sample and readied for Extraction. This stage involves extracting drugs from hair at considerably lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging technique.

Confirmation of any positive Screening findings is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo a wash before confirmation, as necessary. The complete laboratory workflow, from Accessioning to Confirmation, adheres to CAP (College of American Pathologists) Hair requirements and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
