What it is, How it works

When taken near the scalp, hair can offer up to a roughly 3-month window for detecting alcohol and drugs. Hair is easy to collect, harder to tamper with, and simple to send.

A 1.5-inch segment of about 200 hair strands (roughly the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for testing and confirmation. For EtG enhancements and/or analyses beyond 10 panels, 150mg of the sample is advised. We suggest measuring the sample using a jeweler's scale. If scalp hair isn't available, an equivalent quantity of body hair can be taken. When mentioning head hair, we are specifically referring to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The processing of a drug test result in the lab involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into a lab's system. It ensures the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry that might not be provided by an electronic tracking system.

Screening involves a preliminary assessment for drugs of abuse. While it is a cost-effective approach to exclude drug usage in most samples, a positive screening result requires confirmation for it to be legally admissible. Presumptively positive samples in Screening necessitate additional confirmation.

For samples that test presumptively positive in Screening, more hair is extracted from the original specimen for the Extraction phase. Here, drugs are derived from hair at a significantly lower concentration compared to other techniques (such as urine or oral fluid), making hair drug testing the most challenging to execute.

Confirmation for any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The entire lab process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair standard and accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
