If scalp hair is not accessible, an equivalent quantity of body hair can be used. References to head hair pertain solely to scalp hair, whereas body hair includes all other types (such as facial and axillary hair).

Process Overview

The primary steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with the initial processing of a sample within a lab's system. This process involves confirming the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry missing from an electronic chain of custody system.

Screening is conducted as an initial rapid check for substance abuse. While it is an economical approach to dismiss drug usage in most samples, a positive screening must be verified to be acceptable in legal contexts. Samples that show a presumptive positive in Screening need a follow-up confirmation.

Samples with presumptive positives during Screening require additional hair from the original specimen for Extraction. At this stage, drugs are removed from hair at a significantly lower concentration than other methods (like urine or oral fluid), making hair drug screening the most challenging to execute.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptive positives undergo a washing procedure prior to confirmation as applicable. The overall lab procedure from Accessioning through Confirmation is evaluated in line with both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
