Hair analysis has gained recognition as a robust technique for identifying drug and alcohol consumption. The hair shaft effectively captures biomarkers that reflect prolonged exposure to alcohol and drugs. When extracted near the scalp, hair can reveal evidence of drug or alcohol use over a period of up to three months. It is easy to gather, tough to tamper with, and straightforward to transport.

A sample measuring 1.5 inches with roughly 200 hair strands (about the same thickness as a #2 pencil) from the proximity of the scalp yields about 100mg of hair, the preferred quantity for analysis and verification. For EtG, additional tests, or tests exceeding 10 panels, 150mg of hair is advisable. We suggest using a jeweler’s scale to weigh the sample. In cases where scalp hair is missing, an equivalent amount of body hair can be obtained. By head hair, we mean scalp hair, and by body hair, we include all other types (facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory examination of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the beginning of integrating the sample into a lab's framework. This step ensures the sample was appropriately sealed and dispatched, generates a random LAN (Laboratory Accessioning Number), and completes any supplementary data input not included in an electronic chain of custody protocol.

In the Screening phase, the sample undergoes a rapid assessment for drug presence. Although Screening is an economical method to exclude drug consumption in most samples, a positive result requires confirmation before it is considered legally valid. Any samples yielding presumptive positives in Screening necessitate subsequent verification.

For samples indicated as presumptively positive during Screening, additional hair is taken from the original sample for the Extraction phase. In this stage, drugs are isolated from hair at a significantly reduced concentration compared to other procedures (e.g., urine or oral fluid), highlighting the complex nature of hair-based drug testing.

Confirmation of all positive screenings is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all samples marked as presumptively positive undergo washing as necessary. The comprehensive processing, from Accessioning to Confirmation, adheres to rigorous scrutiny under both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation norms.
