If scalp hair isn't an option, a comparable quantity of body hair can be collected. "Head hair" pertains solely to scalp hair. "Body hair" denotes all other types (facial, axillary, etc.).

Process Overview

The four primary stages in the lab analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into a laboratory’s database. This involves checking that it was sealed and transported properly, generating a unique LAN (Laboratory Accessioning Number), and inputting any extra data not provided by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. Although Screening is a budget-friendly approach to rule out drug use in most samples, a positive screen requires confirmation to be permissible in legal settings. Samples that yield a presumptive positive in Screening require subsequent confirmation.

If a sample signals a presumptive positive during Screening, further hair is removed from the original sample and prepared for Extraction. Here, drugs are isolated from hair at a much lower concentration compared to other methods (such as urine or oral fluid), marking hair drug screening as the most intricate method to execute.

Confirmation of positive screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives are washed prior to confirmation when necessary. The complete lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
