What it is, How it works

Hair retains a long-term record of drugs and alcohol use by incorporating biomarkers within the fibers of the developing hair strands. Hair collected near the scalp can provide a detection window for alcohol and drugs lasting up to roughly 90 days. This testing method is straightforward to collect, somewhat resistant to tampering, and simple to transport.

A sample measuring 1.5 inches, consisting of approximately 200 hair strands (similar in size to a #2 pencil) taken closest to the scalp, yields 100mg, which is the optimal sample amount for both screening and corroboration. For tests like EtG, add-ons, or the ones exceeding 10 panels, a specimen quantity of 150mg is advised. It is best to weigh the specimen with a jeweler's scale. Should scalp hair be unavailable, an equivalent quantity of body hair can be collected. When mentioning head hair, it refers exclusively to scalp hair. Body hair encompasses all other types of hair (e.g., facial, axillary).

Process Overview

The four primary stages in the laboratory processing of drug test results encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a sample into a laboratory's system. This step involves confirming that the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and performing supplemental data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid check for substances of abuse. While it is a cost-efficient means to rule out drug use in most samples, a positive screen requires confirmation for legal admissibility. Any samples with a presumptive positive result in Screening must undergo secondary validation.

If a sample shows a presumptive positive result during the Screening stage, additional hair is extracted from the initial sample and prepared for Extraction. At this stage, drugs are extracted from hair at a notably lower concentration than with other methods (like urine or oral fluid), making hair drug testing one of the most challenging methodologies to conduct.

Confirmation of any positive result from screening is achieved using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptively positive samples undergo washing before confirmation as necessary. The full laboratory procedure from Accessioning to Confirmation undergoes review in accordance with both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
