A 1.5-inch section consisting of approximately 200 strands of hair (comparable to the girth of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal amount for both screening and confirmation testing. For EtG enhancements and/or testing beyond 10 panels, a specimen of 150mg is advised. It's suggested to weigh the hair specimen using a jeweler's scale. If scalp hair isn't available, a similar quantity of body hair might be utilized. References to head hair specifically pertain to scalp hair, while body hair encompasses all other hair varieties (such as facial or axillary hair).

Process Overview

The quartet of principal steps in the laboratory evaluation of drug test results includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the beginning stage of processing a sample into a laboratory's system. This phase involves validating the sample's proper sealing and shipment, allocating a random LAN (Laboratory Accessioning Number), and inputting additional data absent from an electronic chain of custody system.

Screening signifies a preliminary inspection for substance abuse. Although Screening provides a budget-friendly method to eliminate drug usage in most samples, any positive findings in a screen require confirmation to be legally valid. Samples presumptively positive during Screening necessitate a further confirmation step.

If a sample suggests positivity in the Screening phase, further hair is extracted from the original specimen and readied for Extraction. During Extraction, drugs are derived from hair in lesser concentrations than other methods (such as urine or oral fluid), rendering hair drug screening a challenging methodology to execute.

Any positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS procedures. All samples suspected to be positive are washed as needed prior to confirmation. The complete laboratory procedure from Accessioning to Confirmation undergoes evaluation in line with the CAP (College of American Pathologists) Hair criteria alongside ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
