A 1.5-inch section of roughly 200 hair strands (similar in size to a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal sample for screening and confirmation. For EtG, additional tests, and/or tests beyond 10 panels, 150mg of the sample is suggested. We recommend using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, a similar amount of body hair might be collected. When head hair is mentioned, it specifically refers to scalp hair. Body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The four primary steps in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial procedure of processing a sample into a lab’s system. This encompasses checking that the sample was sealed and sent correctly, assigning a unique LAN (Laboratory Accessioning Number), and doing any supplementary data entry not covered by an electronic chain of custody system.

Screening is an initial rapid check for drugs of abuse. While it’s a cost-efficient method to exclude drug use for most samples, a positive screen needs confirmation to be valid in a legal setting. Any samples that presumptively test positive in Screening demand a subsequent confirmation.

In the event of a presumptive positive in the Screening phase, more hair is extracted from the original sample for Extraction. During this stage, drugs are extracted from hair at much lower concentrations than in other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screen is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that presumptively test positive are cleansed before confirmation if necessary. The full lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
