For EtG, additional tests, or panels exceeding 10, a specimen weight of 150mg is advised. It is suggested to use a jeweler’s scale for weighing the sample. If scalp hair is not accessible, a comparable quantity of body hair can be used. "Head hair" specifically means scalp hair, whereas "body hair" refers to all other types (beard, underarm, etc.).

Process Overview

The laboratory processing of a drug test result involves four main steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed and entered into a lab’s system. This step ensures the sample was properly sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and completes any remaining data entry absent from an electronic chain of custody.

Screening is a preliminary assessment for drugs of misuse. Though cost-efficient for excluding drug use in most samples, any positive result in Screening must be validated for legal admissibility. Samples identified as likely positive in Screening require further verification.

In cases where Screening indicates a potential positive result, additional hair is extracted from the original sample for the Extraction stage. Here, drugs are extracted from hair at substantially lower concentrations compared to other methods such as urine or saliva analysis, underscoring why hair drug screening is the most challenging method to perform.

Any positive Screening outcomes are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. Before confirmation, all presumed positive samples undergo washing as needed. The complete lab procedure from Accessioning to Confirmation is scrutinized under the College of American Pathologists Hair classification and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
