What it is, How it works

When extracted near the scalp, hair can offer an approximate 3-month detection timeframe for alcohol and drugs. Hair collection is straightforward, somewhat challenging to tamper with, and convenient to transport.

A 1.5-inch section of roughly 200 hair strands (comparable to a #2 pencil) nearest to the scalp will yield 100mg of hair, the perfect amount for both screening and confirmation. For EtG, supplemental tests, and/or assessments beyond 10 panels, it is advised to use 150mg of the sample. We advise measuring the specimen with a jeweler’s scale. If scalp hair is not accessible, an equivalent quantity of body hair may be used. Reference to head hair specifically means scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The principal stages involved in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial handling of a sample in the lab's system. This step includes ensuring the sample is sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not available via an electronic chain of custody system.

Screening involves a preliminary analysis for drugs of abuse. While Screening is a cost-efficient method to eliminate drug usage for most samples, a positive screen must be confirmed to be valid for court. Any samples that are deemed presumptively positive in Screening require further confirmation.

If a sample shows a presumptive positive in the Screening phase, more hair is taken from the initial specimen for Extraction. During this phase, drugs are extracted from hair at much lower concentrations than in other testing methods (e.g., urine or oral fluid), highlighting why hair drug screening is the most complex method to carry out.

Confirmation of any positive screening outcome is performed with GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are washed as necessary. The entire laboratory process, from Accessioning to Confirmation, undergoes a review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
