If head hair is not obtainable, an equal amount of body hair may be used. When mentioning head hair, it specifically pertains to scalp hair, while body hair includes other types such as facial and axillary.

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is integrated into a lab's system. This step ensures the sample was properly sealed and shipped, assigns it a Laboratory Accessioning Number (LAN) at random, and completes any missing data entry not addressed by an electronic chain of custody system.

Screening offers an initial quick assessment for substance abuse. While cost-efficient for excluding drug presence in most samples, a positive screen must undergo verification to be legally admissible. Presumptively positive samples necessitate further confirmation.

If a sample tests positive during the Screening phase, additional hair is extracted from the original sample for the Extraction process. This step separates drugs from the hair at much lower concentrations compared to other methods like urine or oral fluid testing, making hair screening a challenging procedure to undertake.

Any positive screening outcomes are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to final confirmation, samples are also cleansed as required. The entire laboratory process, from Accessioning to Confirmation, is audited under both CAP (College of American Pathologists) standards and accredited by ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
