What it is, How it works

When sampled near the scalp, hair can reveal a detection window of up to 3 months for alcohol and drugs. Hair collection is straightforward, relatively tamper-resistant, and convenient for shipment.

A 1.5-inch hair sample comprising roughly 200 strands (approximately the width of a #2 pencil) taken nearest to the scalp equates to 100mg, which is the optimal sample size for preliminary and confirmatory testing. For EtG, additional tests, and/or panels exceeding 10, a 150mg specimen is suggested. Utilizing a jeweler’s scale for weighing the specimen is advised. If scalp hair is not present, an equivalent quantity of body hair may be obtained. Head hair specifically refers to scalp hair. Body hair encompasses all other types of hair (facial, underarm, etc.).

Process Overview

The crucial stages in analyzing a drug test sample within a laboratory context include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary integration of a sample into a lab’s system, verifying proper sealing and shipment, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary supplemental data entry absent in an electronic chain of custody system.

Screening operates as an initial swift assessment for substances of abuse. Screening is an economical method to exclude drug consumption for most samples, however, a positive screen necessitates confirmation for legal acceptability. Any samples with a presumptive positive in Screening require a follow-up confirmation.

If Screening indicates a presumptive positive, additional hair from the original sample is utilized for Extraction. In this phase, drugs are derived from hair at much lower concentrations than alternative methods (e.g., urine or oral fluid), which makes hair drug screening especially challenging.

Positive results from Screening undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Samples with presumptive positives are cleansed before Confirmation when required. The complete lab process from Accessioning to Confirmation adheres to CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
