If scalp hair is not available, an equivalent quantity of body hair can be collected. References to head hair specifically mean scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The four principal steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial integration of a sample into the lab's system. This step includes verifying the sample’s proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not captured by an electronic chain of custody system.

Screening entails a preliminary rapid examination for drugs of abuse. Although Screening is an economical way to eliminate drug presence in most samples, a positive result requires confirmation to be court-admissible. All samples that test positive during Screening necessitate secondary confirmation.

If a sample tests presumptively positive in the Screening phase, more hair is extracted from the original specimen to prepare for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration compared to other methods (such as urine or oral fluid), which makes hair drug screening the most challenging to execute.

Positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. All samples identified as presumptive positives are washed prior to confirmation as deemed necessary. The complete laboratory procedure from Accessioning to Confirmation follows review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
